This chronic disease self management program follows a set procedure for giving strategies to deal with the emotional aspects of having a chronic condition. It does not look at disease or medical specifics but in an interactive way encourages participants to make active self management decisions about things that impact on their health and well being. The program is led by specially trained people who either have an ongoing condition themselves or are a supporter / carer for someone with an ongoing health condition. The program is run by Department of Health & Human Services but information is available through Arthritis Tasmania
